UWCF’s mission is to inspire our community to give, advocate and volunteer by identifying needs and mobilizing vetted resources to create positive, local change. We envision a united community where all are empowered to achieve their fullest potential. Annually, we provide services to more than 186,867 individuals in Central Florida, through our four Impact areas: Early Childhood Education, Academic Achievement, Financial Stability, Health, End Hunger, Community Partnership School, and Basic Needs.
UWCF has long worked with local health experts to identify the health services that would meet local, critical need and the specific assessments that would reliably measure the results of funded services.
UWCF is partnering with Polk County Health and Human Services Community Health Care program to bring new opportunities to qualifying residents of Polk County, for behavioral health programs/services and prevention/early intervention programs/services to individuals and family members, as well as healthcare coverage.
In conjunction with Polk County Health and Human Services Community Health Care (CHC) program administration, United Way of Central Florida has $10 million available for the UWCF Community Wellness Initiative 4th year program funding (Florida Statutes 212.055 (7); Polk County Ordinance 2015-76, as amended). UWCF desires to contract with 501(c)(3) agencies/organizations to provide programs/services for behavioral health and prevention/early intervention, to reduce health care disparities to individuals and families in Polk County.
UWCF requests proposals for new programs/services, expansion of existing programs/services, and/or equipment. Multi-year funding is possible for programs/services.
The initiative’s focus is to serve Polk County residents without healthcare insurance/coverage with incomes at or below 200% of the Federal Poverty Level.
With this initiative, we are proposing to align community-wide efforts to achieve greater impact through collaboration through the following objectives:
- Prevention/Early Intervention
- Behavioral Health
